What to expect at your first appointment
Parents and guardians: Please arrive 15 minutes before your child’s scheduled appointment. Be sure to bring the following documents:
- Photo ID of parent or legal guardian
- Your child’s insurance card, if they have one
- Your child’s immunization records, if they have them
For all patients: New patients will need to complete some paperwork at your first visit. To save time you can print and fill out the new patient forms in advance.

We accept Medicaid/CHIP/CHIP Perinatal, Medicare, Marketplace coverage, and some private insurance plans. Please call us at 713-695-4013 to confirm we can accept your specific plan.
No insurance? No problem.

Need to cancel? It is easy.
It is very easy to cancel an appointment. Just respond “No” to any texts we send you to confirm your visit. Or, call us at 713-674-3326.
When you know you will not be able to keep a scheduled visit, it is vital to let us know so that the appointment can be made available to another patient who needs it.
