What to Expect at Your First Appointment

Please arrive 15 minutes before your scheduled time at your first appointment. You’ll need to provide the following documentation :

  • Photo ID of parent or legal guardian
  • Your child’s insurance card, if he or she has one
  • Your child’s immunization records if he or she has them

Also, there’s some paper work for new patients that you’ll need to complete when you arrive. You can print and fill out the new patient forms ahead of time, if you wish. To download, click on the form titles below.


Payment Options


We accept Medicaid/CHIP/CHIP Perinatal, Marketplace, and some private insurance plans. Please call to make sure we accept your insurance.

Family Assistance Program (Sliding Fee Discount)

If you don’t have health insurance, ask about our Family Assistance Program, for sliding fee discounts that are available to those who qualify. To apply, arrive 45 minutes before your scheduled time to your first appointment.

Need to cancel?

If you are unable to keep your appointment, please call 713-674-3326 and cancel 24 hours in advance so another patient can be given your time.
