What to Expect at Your First Appointment
Please arrive 15 minutes before your scheduled time at your first appointment. You’ll need to provide the following documentation :
- Photo ID of parent or legal guardian
- Your child’s insurance card, if he or she has one
- Your child’s immunization records if he or she has them
Also, there’s some paper work for new patients that you’ll need to complete when you arrive. You can print and fill out the new patient forms ahead of time, if you wish. To download, click on the form titles below.
We accept Medicaid/CHIP/CHIP Perinatal, Marketplace, and some private insurance plans. Please call to make sure we accept your insurance.
Get Care Program — Family Assistance (Sliding Fee Discount)
If you don’t have health insurance, ask about our Get Care Program, which includes our Family Assistance with sliding fee discounts available to those who qualify. We can also help you understand other benefits available to you. To apply, arrive 45 minutes before your scheduled time for your first appointment.
Need to cancel? It is easy.
It is very easy to cancel an appointment. Just respond “No” to any texts we send you to confirm your visit. Or, call us at 713-674-3326.
When you know you will not be able to keep a scheduled visit, it is vital to let us know so that the appointment can be made available to another patient who needs it.
